What is Social Prescribing?
Social Prescribing is a new free support service offered by Shannon Family Resource Centre which aims to reduce social isolation and improve a person’s health and wellbeing. Our Social Prescribing Link worker will support you to access local services, such as social groups, exercise classes, relaxation sessions, volunteering opportunities, support services and much more in your local area.

Who is Social Prescribing For?
Social Prescribing is for any person over the age of 18, that wants to improve their quality of life.

- Who are living with one or more long term health conditions.
- Who need support with their mental health.
- Who are lonely or isolated.
- Who are frequent GP attendees and may benefit from other social supports outside of clinical services.
- Who have complex social needs which affect their health and wellbeing.
- The service is suitable to people who are ready for a change.
How does Social Prescribing work?
Step 1: You can contact our Social Prescribing Link Worker Suzanne at Shannon FRC yourself or be referred by your GP/Doctor, family member or friend.
Step 2: Our Link Worker Suzanne will arrange to meet with you.
Step 3: Suzanne will listen to what you are interested in and work with you to provide information on the local service of your choice.
Step 4: You will choose the service and supports that are right for you.
Step 5: Our Link Worker will meet with you for 6- 8 sessions to ensure you are happy in the activity of your choice and to ensure your experience is a happy one.
